Are you a Registered Nurse who loves teaching as much as clinical care?

Our client is looking for a Home Training Nurse to lead their home dialysis program in Louisville. Unlike the fast-paced in-center environment, this role focuses on one-on-one education, where you will coach patients and their care partners to safely perform Peritoneal Dialysis (PD) or Home Hemodialysis (HHD).

Key Responsibilities

  • Patient Education: Conduct comprehensive training programs for patients and caregivers on the safe operation of home dialysis equipment.
  • Case Management: Oversee a caseload of home patients, performing monthly clinic visits and regular home assessments.
  • Clinical Assessment: Evaluate patient readiness for home therapies and develop individualized training plans based on their learning style.
  • Technical Support: Troubleshoot equipment issues and provide clinical guidance to patients via phone or in-person visits.
  • Collaboration: Act as the primary liaison between the patient and the Nephrologist to ensure treatment adequacy and safety.

Qualifications

  • Licensure: Active, unencumbered RN License in Kentucky (or compact state).
  • Experience: With at least 2 years of experience in Dialysis (PD or HHD)
  • Skills: Strong "teacher's heart" with the ability to explain complex medical procedures in simple, layman's terms.
  • Compliance: Thorough understanding of CMS regulations regarding home therapy documentation and safety.
