Our client is seeking a dedicated Home Health Physical Therapist to join their multidisciplinary team in Grants Pass. This role allows you to step out of the high-volume clinic setting and into the patient's real-world environment, where you can design truly functional recovery plans that make a measurable difference in their daily lives.

Key Responsibilities

  • Comprehensive Assessments: Evaluate patient functional status, mobility, and safety within their home environment.
  • Treatment Planning: Develop and implement individualized plans of care aimed at improving strength, balance, and independence.
  • Functional Rehabilitation: Focus on real-world skills, such as fall prevention, gait training, and home exercise program education.
  • Collaboration: Coordinate with an interdisciplinary team (RNs, OTs, and Physicians) to ensure a holistic approach to patient recovery.
  • Documentation: Utilize electronic medical records (EMR) to maintain timely and accurate progress notes in accordance with CMS regulations.

Qualifications

  • Licensure: Active, unencumbered Physical Therapist (PT) license in the state of Oregon.
  • Education: Graduate of an accredited Physical Therapy program (DPT preferred).
  • Certification: Current BLS (American Heart Association) required.
  • Experience: 1+ year of clinical PT experience preferred; home health experience is a major plus but not required for candidates with strong acute or rehab backgrounds.
  • Mobility: Must have a valid drivers license, reliable transportation, and proof of auto insurance.
