Job Description:

Our client is seeking a full-time Home Health Physical Therapist (PT) to join their growing Waco branch. This is a chance to move away from the high-volume, "conveyor belt" feel of a traditional clinic and focus on delivering high-impact, one-on-one care where it matters most: in the patient's home. You will be responsible for helping patients recover from surgery, injury, or chronic illness while enjoying the professional autonomy that only home health provides.

Key Responsibilities

  • Evaluations & Plans of Care: Perform initial physical therapy evaluations and develop individualized, goal-oriented treatment plans.
  • Functional Rehabilitation: Implement therapeutic exercises, gait training, and balance retraining focused on real-world home safety.
  • Interdisciplinary Collaboration: Work closely with RNs, OTs, and Physicians to ensure a comprehensive approach to patient wellness.
  • Patient & Family Education: Train caregivers and patients on home exercise programs (HEP) and the safe use of assistive devices.
  • Documentation: Utilize mobile EMR technology to document visits and progress in real-time, ensuring compliance with CMS standards.

Qualifications

  • Licensure: Active, unencumbered Physical Therapist (PT) license in the state of Texas.
  • Education: Graduate of an accredited Physical Therapy program (DPT preferred).
  • Clinical Experience: 1+ year of PT experience is preferred; home health experience is a plus but not required for candidates with strong clinical skills.
  • Driver Requirements: Valid drivers license, reliable transportation, and active auto insurance.
  • Compliance: BLS certification and ability to pass a background/drug screen.
